Depuis près de 40 ans, des chercheurs du CGS ont pour objet de recherche la dynamique du secteur de la santé et explorent son mode de transformation. L'émergence de la « démarche gestionnaire » à l'hôpital a donné lieu à de multiples travaux du CGS, et se poursuit encore aujourd'hui. Le CGS accompagne, à travers des travaux de doctorants ou de chercheurs séniors, plusieurs évolutions majeures du secteur (ruptures de parcours, coordination ville-hôpital, montée en puissance de la logique de parcours et de la logique de prévention, impact organisationnel des nouvelles technologies en santé, mode de financement des hôpitaux, évolution de la T2A, usages de l'IA en santé, virage écologique du secteur de la santé,…)
L'irruption du numérique dans le secteur de la santé apparait constituer un levier majeur de transformation du secteur. Elle permet des nouveaux modes de prise en charge des patients (télésurveillance, téléexpertise, IA pour le diagnostic,…), et facilite des évolutions majeures comme le virage de la prévention, en favorisant la collaboration
entre plusieurs acteurs (hôpitaux, start-up, professionnels de ville, associations de patients, chercheurs,…).
Mais la question de l'organisation de ce nouvel éco-système du numérique en santé se pose, tant les acteurs s'inscrivent dans des champs de travail cloisonnés et dispersés, mettant en avant des enjeux de coordination considérables. Se pose également la question de la valorisation socio-économique de ces données et des applications qui en sont issues, qui loin de se produire spontanément, appelle à développer des formes de « co-design » spécifiques qui aident les acteurs à concevoir les nouveaux produits et nouvelles applications, ainsi que les nouveaux usages des données collectées.
C'est ces deux enjeux, organisationnels et numériques, que la thèse se propose d'étudier.

For almost 40 years, CGS researchers have been investigating the dynamics of the healthcare sector and exploring its transformation. The emergence of the “managerial approach” in hospitals has been the subject of numerous CGS projects, and continues to be so today. Through the work of doctoral students and senior researchers, the CGS has accompanied a number of major developments in the sector (disruptions in care, city-hospital coordination, the growing importance of care pathways and prevention, the organizational impact of new healthcare technologies, hospital financing methods, changes in T2A, the use of AI in healthcare, the ecological shift in the healthcare sector, etc.).
The irruption of digital technology in the healthcare sector appears to be a major lever for transforming the sector. It is enabling new ways of caring for patients (remote monitoring, tele-expertise, AI for diagnosis, etc.), and facilitating major developments such as the shift towards prevention, by encouraging collaboration between several
players (hospitals, start-ups, healthcare professionals, patient associations, researchers, etc.).
But the question of how to organize this new digital health eco-system arises, given that the players involved work in compartmentalized and dispersed fields, highlighting considerable coordination challenges. There is also the question of the socio-economic value of this data and the applications derived from it, which far from occurring spontaneously, calls for the development of specific forms of “co-design” that help players to design new products and new applications, as well as new uses for the data collected.
It is these two issues - organizational and digital - that this thesis will examine.

- Methods for supervising and monitoring the PhD student's training and research progress :
- on-site support for a hospital group on these full-scale projects, involving start-ups
- supervision by specialized teaching and research staff
- interviews with decision-makers and industry players
In keeping with the CGS tradition, work is carried out in collaboration with players from the healthcare world, with a strong focus on specific projects carried out by hospitals.
